• Completed certificate request form.
• One photo photo (4 x 6cm or 2 x 2in, white background, front-facing, bare head, clear face and ears, no glasses). Photo Sample.
• Copy/photo of U.S. passport.
• One of the following documents (including documents issued by the former government of South Vietnam): birth certificate, national ID card, citizen ID, household registration book, or old Vietnamese passport — to confirm the current or past Vietnamese citizenship.
• Contact information (address, phone number, email, full name of the person receiving the documents).

Note:
If the name on Vietnamese birth certificate is different from the current name, a legal name change certificate is required.
