Applicable to individuals requesting to bring the remains or cremated ashes of a deceased person from the United States to Vietnam for burial or safekeeping permitted by Authority in Vietnam and by Vietnamese Embassy/Consulate General in the United States in accordance with the provisions of Vietnamese law.

1. Required Documents

• Completed Application form to request permit for transporting remains or ashes to Vietnam.
• Personal identification documents of the applicant and, if applicable, documents proving the family relationship with the deceased.
• Documents verifying that the deceased is eligible for repatriation of remains to Vietnam:
  • Copy/photo of the deceased’s passport or any travel documents (if available), or
  • Copy of permanent residence permit in Vietnam (if the deceased is a foreign national).
  • Certified copy of the death certificate issued by a competent foreign authority or consular office.
  • Certified copy of the sanitary and health quarantine certificate issued by a competent foreign authority (for remains).
  • Exhumation certificate and sanitary/quarantine certificate (for remains).
  • Cremation certificate (for ashes).
• Copy/photo of the written approval from the local People’s Committee in Vietnam or cemetery management agency allowing the repatriation and burial/safekeeping, if the deceased is a Vietnamese citizen residing abroad.
• Contact information (address, phone number, email, full name of the person receiving the documents).
