1. Required Documents

A 4cm x 6cm or 2 x 2in passport photo (white background, front-facing, bare head, clear face and ears, no glasses).
A copy/photo of documents proving legal residence in the U.S. (Green card, U.S. passport, work permit, valid visa, etc.)
A copy/photo of documents proving Vietnamese nationality (travel document, birth certificate, old Vietnamese ID card, household registration book, graduation certificate, if any).
Completed personal background declaration form.
Contact information (address, phone number, email, full name of the person receiving the documents).

3. Important Notes

Processing time starts once we receive the complete application and it is accepted by the Consulate General.
